Changing Providers — Future Contributions

Participants may change ORP Providers as the recipient of future contributions anytime after twelve months have elapsed since the more recent of:

1. their initial enrollment, and
   
   
2. their last change.

To change Providers for prospective contributions, you must submit these completed forms to your Benefits Administrator:

• ORP Enrollment/Change Form: indicate that this is a “Change of Provider” and select your new Provider; and
  
  
• Provider Application (AIG Retirement, Lincoln or TIAA-CREF).

Reminder: Changing the destination of future contributions (to a new Provider) does not automatically transfer current assets between Providers; nor does transferring current assets between Providers effect a change in Providers receiving future contributions.
