ORP Statement for Social Security
ORP Administratorís Statement: The ORP Plan Administrator provides a statement of your participation in the ORP; your account balance on the day you terminated employment with the Commonwealth; and a confirmation of your ORP Provider for you.
You must present the Statement to the SSA Representative who submits your application for retirement benefits.
The Plan Administrator will prepare your Statement when you request it.
