Service members

Download and submit a completed VA Form 22-1990 [.PDF], Application For VA Education Benefits, or apply online using VONAPP. The VA may request a DD-214 from veterans, or a Notice of Basic Eligibility from active Guardsmen or Select Reservists.

Dependents

Submit a completed VA Form 22-5490 [.PDF], Application For VA Education Benefits, or apply online using VONAPP.
