ORP Administrator’s Statement: The ORP plan administrator provides a statement of your participation in the ORP; your account balance on the day you terminated employment with the Commonwealth; and a confirmation of your ORP provider for you.
You must present the statement to the SSA representative who submits your application for retirement benefits.
The plan administrator will prepare your statement when you request it.
Contact the plan administrator: You must contact the plan administrator to request your statement if you are applying for Social Security benefits and have not yet received one.
